I will monitor my player for any symptoms of COVID-19 before bringing them to an arena for any Meadow Lake Minor Hockey event.

I agree that I will not bring my Player(s) to any Meadow Lake Minor Hockey event or events if they are showing any signs of COVID-19

I will contact the Meadow Lake Minor Hockey Office and my Coaches immediately if anyone in my family has been tested form COVID-19 or has been exposed to someone that has tested positive.

I will follow the COVID-19 Contact Tracing Protocol that will be set in place at all MLMH events. I acknowledge that if the rules have not been followed I will be asked to leave the arena.

I understand that my player(s) may be playing on a team with players or against a team that has players that live in the surrounding communities.

I will follow any rules put in place by the City of Meadow Lake. As well as all the rules put in place by the Saskatchewan Government, Saskatchewan Health Authority, Saskatchewan Hockey and Hockey Canada.

I acknowledge that I will not hold Meadow Lake Minor Hockey responsible if any members of my household contacts COVID-19
